Information
Medfest is a free opportunity for athletes to come get a medical exam in order to complete the Athlete Registration and Medical form. This form must be completed every 3 years in order for an athlete to participate in practice and competition.
First, fill out the Athlete Registration and Medical Form online. Then follow the instructions to print page 3 and bring it to the Training for Life Center.
